Tenma Kenzo, a Japanese working at the West German Eisler Memorial Hospital, is known as a "genius doctor". He has superb surgical skills and is humble. He is envied by his colleagues and respected by doctors and patients. In his career, Tenma has a bright future and is favored by the dean; in love, the dean's daughter Eva is in love with him and they are inseparable. However, Tenma, who seems to have everything good, gradually feels uneasy in his conscience. He witnessed with his own eyes that the hospital ignored ordinary people in critical condition in order to rescue important people. The accusations and crying of the families of the deceased made Tenma suffer. When this dilemma comes again, Tenma resolutely chooses to rescue a boy who was shot in the head.
